I think you could probably (not sure of interactions with other meds) combine kava kava, psilocybin, cbd, and cbg and you'd get a decent amount of relief. Kava is underrated.
IF you were to incorporate kratom, do it blind. Meaning have the dose precisely measured and have it added to something (like the above concoction which makes your mouth numb so it'd be undetectable) without your knowledge, only on the days and at the time when pain is the worst. But again, check to make sure you can take it safely with a doctor. If I were in your shoes I'd want it completely hidden from me.
But honestly dude, your kind of pain is exactly what pharma opiates are for. There's no shame in taking them when it's needed. I was a "never opiates again" guy after they killed my sister... until I broke my ankle in 5 places and was screaming from the pain. They have their place.
Again, be careful. The nature of opiates is that they will invent pain for incentive to take more. The only thing that makes kratom different from pharma opiates is the lesser OD potential. And the potential for heavy metal poisoning. And the lack of testing that whatever's in the bag is pure. And the fact that it can turn your skin a weird bluish gray color and make you irritable.

Also, don't underestimate the effectiveness of topical ointments. I'm constantly in pain and the Chinese market by my house is so clutch. Icy Hot and Ben-gay are solid, but better are Zheng Gu Shui, Po Sum On, White Lotus Oil, and Zang Hong Hua. And Tiger Balm, obviously. I just finished putting some of the first two and the resin from my vape on my knee, which got hit decently with another knee in Muay Thai class tonight and has a nice Grimace-purple bruise coming up.
