Somaclones

The explants may genetically modify during culture. It's not common but not impossible. I know that a lot of companies therefore fingerprint in and fingerprint out to confirm genetic fidelity.

I'm sure that these types of genetic abnormalities are more common when you are doing meristem work as opposed to nodal. Remember that a meristematic dome is a small cluster of cells about 0.1mm across and quite a bit more fragile and manipulatable than an apical tip or a node.
